PATIENT’S RIGHTS
Dignity:
You have the right to considerate and respectful care.
Informed Consent:
This means that you have received complete and current information about diagnosis, treatment, risks and possible
outcomes from your doctor in terms that you can understand and that you agree to medical procedures proposed by your doctor.
Staff Identification:
Our employees wear identification. You have the right to know who is taking care of you. Your doctor and consulting
doctors will also tell you their names.
Privacy:
We will do everything we can to maintain your privacy. All medical procedures, communications and records will be kept
confidential and released only according to guidelines of HIPAA (Health Insurance Portability and Accountability Act).
Treatment:
All patients have the right to medical care regardless of race, sex, creed, color, or national origin.
Research, Education or Experimental Projects:
If the center is involved in clinical research that might involve you, we will inform you in advance. You have the
right to refuse such treatment.
Billing Procedures:
You have the right to receive an explanation of each item on your bill.
Decisions:
We want you to be fully aware of procedures prescribed for you. Patients have the right to consult with a specialist
upon request and at the patient’s expense. You also have the right to refuse treatment to the extent permitted by law.
We feel a responsibility to respond to your requests regarding continuity of care. If we feel that moving you to another
facility is for your well-being, you will be informed and will receive an explanation about the need for transfer and
any arrangements that have been made.

Patient’s Responsibilities
So that you will receive the best treatment possible, you should provide accurate and complete information about present
complaints, past illnesses, hospitalizations, medications and other matters related to your health.
Your doctor works with the center to develop your treatment plan and instructions. You are responsible for following
this plan. You are responsible for your actions if you refuse treatment or do not follow instructions.
You are responsible for assuring that your financial obligations to the center are fulfilled as promptly as possible
and for giving us information needed to process insurance.
All patients are responsible for following center rules and regulations affecting patient care and conduct.
